To complete your registration, please print the Baseball Registration Form/Medical Release also located at the left. Complete the form, sign the Medical Release, mark the credit card box, fill in with the words PAID ON LINE and mail the form to the Minnetonka Diamond Club at PO Box 573, Excelsior, MN 55331.
